According the National Center for Chronic Disease Prevention and Health Promotion
6 in 10 adults in the US have a chronic disease.  Chronic disease increases the risk of hospitalization and chronic diseases account for 90% of the annual US healthcare expenditures. Often the implications of chronic conditions can limit an individual's ability to participate in meaningful occupations.  Self-management is an approach that works to educate individuals about their condition and helps them to  become active participants in their treatment.

This course will discuss self-management in treating chronic conditions and help occupational therapy practitioners understand the distinct value they bring to addressing self-management of chronic conditions in a variety of settings such as acute care, inpatient rehabilitation, skilled nursing, home health, outpatient, school, work and the community.

Target Audience: Occupational therapists, occupational therapy assistants, occupational therapy students

This occupational therapy continuing education course uses the following instructional methods: visual aides (ie slides), discussion board, guided note taking, and learning, and practice opportunities.

Content disclosure: This course does not exclusively focus on any specific product or service.

What OTs and OTAs are saying about this course

I took a chance on this course since chronic conditions are not frequently relevant in my setting however I was impressed with what I learned and how it could be applied in my setting!

OT Course Participant

I feel like it has given me a fresh approach on options/interventional strategies I can use with my patients, most of whom have chronic conditions. I also feel very empowered after participating in this course.

OT Course Participant

I felt this course was very informative and that the knowledge and skills are transferable to even other areas in life (general goal setting/life mgmt.).
